AI in ECG, EchoNext, and Digital Biomarkers

 Artificial intelligence (AI) has been making waves in various fields, and heart health is no exception. One of the most exciting advancements is in the realm of early heart disease detection. For decades, doctors have relied on ECGs (electrocardiograms) to monitor heart function and identify potential issues. However, these tests require human interpretation, which, despite the expertise of cardiologists, is not foolproof. The emergence of AI models capable of analyzing ECGs with high accuracy is transforming this landscape.

One such breakthrough is EchoNext, a new AI model developed by researchers at Columbia University. EchoNext is designed to analyze standard ECG readings, identifying heart disease that might otherwise go unnoticed. In clinical testing, EchoNext has outperformed cardiologists in diagnostic accuracy, spotting issues like heart valve dysfunction and thickened myocardium (the heart muscle) in patients. The AI model demonstrated 77% accuracy in detecting heart disease, compared to 64% for human experts.

But what makes EchoNext particularly impressive is its ability to flag cases that might otherwise be missed by traditional methods. It allows doctors to catch potential heart issues early on, giving patients a better chance of receiving timely interventions. This is just the beginning of a broader trend where AI is revolutionizing preventative cardiology, moving us toward a future where routine tests like ECGs are not just diagnostic tools but early warning systems.

In addition to EchoNext, another area where AI is making an impact is in digital biomarkers. These are data points generated by wearable devices such as smartwatches or fitness trackers, which continuously monitor heart health. One recent innovation involves AI-driven models that can estimate a person's “vascular age” based on signals from wearable devices. The concept of vascular age refers to how well the blood vessels are functioning in relation to a person’s actual age.

In a recent study, AI models analyzed data from photoplethysmogram (PPG) signals—a technology found in many fitness trackers—to estimate vascular age. The results were startling: individuals with a vascular age that was more than 9 years older than their chronological age were found to be at increased risk of heart disease, stroke, diabetes, and even premature death. This highlights the potential of AI to predict heart health risks before they become evident through symptoms.

The integration of AI into heart health diagnostics is set to revolutionize the field. By using everyday technology like wearables, AI will empower individuals to keep track of their heart health in real-time, leading to earlier intervention, personalized treatment, and, ultimately, healthier lives.
